ITS will be implementing a new Access Request Form. This new form will be your “one stop” for access on campus. Here are some of the changes to the form:

• New field for contract users such as UNICCO, Bon Appetite.
• New field for requesting Housing/Student Info access.
• Two new fields for ID card access and key access.
• Banner access now requires that a separate Banner System Request Form accompany the main Access Request Form.
• Random passwords will now be generated for all systems and Desktop Support will walk you through making the initial password change.

These changes have been implemented so that we only need one form and one supervisor signature in most cases. The Banner System Request Form has been recreated to give more detail of the modules and the access requested.
